CONNECTMYHEALTH PATIENT PORTAL

To support Ontario’s plans to introduce new means for more Ontarians to access their personal health information, the HITS eHealth Office, in collaboration with Ontario Health Teams in Ontario Health’s West Region, has received approval and funding from Ontario Health to productionize and deploy a regional patient portal called ConnectMyHealth. ConnectMyHealth is the patient-facing version of ClinicalConnect, the clinical viewer used by healthcare providers in Ontario Health West, and provides patients in this region with an online, single access channel to view their health records from various healthcare facilities in Ontario.
